Appointments

What you need to know

Office visits are made by appointment only. Since injuries and urgent problems are unpredictable, we set aside several time slots each day for these situations. Problems of this nature are seen the same day or next day as necessary.

If an office appointment needs to be cancelled/ rescheduled or you may be delayed for your appointment, please call the office as soon as possible. We will make every effort to accommodate your schedule. A fee will be charged for appointments that are not kept and not cancelled.

If this is your first visit with us, for your convenience, you may download and complete the Patient Registration and Medical History forms before your visit. Please bring these completed forms with you at your visit. Or, they can be mailed to either office location where you are being seen or faxed to 434-977-8083 prior to your visit.


In case of emergency

For emergent problems during office hours, please call our office immediately. We will instruct you depending upon the nature of your problem.

For emergent problems after office hours or weekends that require immediate medical care, you will be directed to the Emergency Department at either Martha Jefferson Hospital or Culpeper Regional Hospital.


Coverage

Our practice provides coverage for our patients 24 hours per day, 7 days per week.

In the event of a medical problem where you need to speak with a doctor, please call our office day or night and you will be directed to the covering doctor.


Insurance plans accepted

We participate with most major medical care plans. Please contact our Patient Care Coordinator at (434) 977-8040 for further details on insurance coverage and referral issues.

Note: Health plan participation is subject to change. Please contact the office to clarify health plan information.


Payment information

All professional fees are based upon services provided. As a service to our patients, we will file your insurance claims with your primary insurer. All deductibles, co-payments, and cost of supplies will be required at the time of service, unless prior financial arrangements have been made.

For your convenience, we accept the following forms of payment:

Prescriptions and renewals

Please call our office during regular business hours for prescription renewals. Please have the name and telephone number of your pharmacy available when you call.

Copies of medical records and x-rays

Copies of your medical records are available upon request. In order to receive a copy of your records the completion of a Request of Medical Records Form is required. This process can be completed in our office. Or, you may download this form from the Forms and Instructions Page, please complete the form and mail to our office. Preparation and shipping fees will apply.

Please allow 3 business days for preparation of your records.


Completion of disability forms

Disability forms will be completed for you by submitting these forms to our office. Please complete the patient information section that applies to you prior to submitting this form to our office. A disability form preparation fee will apply per form.

Please allow 3 business days for preparation of these documents.


Returned check for insufficient funds

A service fee will be charged for checks that have been returned for insufficient funds.
